Community Treatment (ACT) uses a multidisciplinary team approach with assertive outreach in the community. The consistent, caring, person-centered relationships have a positive effect on outcomes and quality of life for participants.
This initiative will serve, annually, 40 native adults diagnosed with Serious Mental Illness (SMI) with a case manager and peer support specialist who will connect them to various community services to include job readiness, housing assistance, education, and life skills development.
